The easiest way to add a second fly in a tandem rig is to attach the dropper line directly to the hook of the top fly. However, this is not producing the best results. A lot of anglers consider The surgeon's knot dropper a quick and easy and better way to add a dropper.

With small tippets, 7X, is recommended to use a triple surgeon knot.

Sometimes this knot is the weakest point in our rig, but there are some actions to improve it.

It is better to modify the knot after tying the dropper, in order to position the dropper perpendicular to the main tippet. This will allow the dropper to avoid tangle. But more important than that, the tension will be transmitted straight from the fly to the leader, no matter which fly the trout takes. This knot will place the point fly perpendicular to the main tippet when there is take in the dropper, and it places the dropper perpendicular to the main tippet when there is a take in the point fly.

This transmits the tension directly from the fly taken by the fish to the leader. This modification is important because it protects the weakest point of the rig.
